The Thyroid Awakening Panel and Heal Your Gut, Free Your Thyroid programs with wellness-based functional lab panels offered for informational and educational purposes only. It is not intended to diagnose, treat, cure, or prevent any disease, nor to replace any consultation with a licensed medical provider.

By purchasing this lab, you understand and agree that:

  • This panel is not a medical evaluation, diagnostic test, or treatment.
  • No doctor–patient relationship is created by purchasing the lab or participating in consultations.
  • All interpretations are provided as educational insights into general wellness patterns, not as medical advice.
  • You should consult your licensed physician or healthcare provider for any questions regarding diagnosis, treatment, medication, or medical conditions.
  • You are voluntarily choosing to purchase this lab for personal education and wellness optimization.
  • Results and recommendations are not a substitute for medical care.

Purchases are non-refundable once the lab requisition has been submitted.

All personal health information is kept confidential using HIPAA-aligned privacy practices.

Purpose of Services  ::  I understand that the Thyroid Awakening Panel and any associated consultations, educational materials, or follow-up discussions are provided for informational and educational purposes within a functional nutrition and wellness framework. These services are not intended to diagnose, treat, prevent, or cure any disease.

Scope of Practice  ::  I understand that Dr. Joni Labbe, DC, CCN, DCCN, provides services under a functional health, nutritional consulting, and wellness education model, not as a treating physician. While she holds Dr. of chiropractic and clinical nutrition credentials, these services do not constitute medical treatment, or diagnosis.

No Medical Relationship Created  ::  I understand that completion of this form, purchase of the lab panel, consultations, or communication with Dr. Labbedoes not create a doctor–patient relationship. All medical care, diagnosis, and treatment decisions remain under the care of my licensed medical provider(s).

Limits of Recommendations  ::  I understand that any recommendations provided may include lifestyle, nutritional, dietary, behavioral, or supplement education, and are not medical prescriptions or treatment plans.
Dr. Labbe will not:

  • Diagnose medical conditions
  • Advise about prescription medications
  • Adjust thyroid or other medications
  • Claim that any supplement, food, or strategy will cure a condition

Voluntary Participation  ::  I am choosing to participate voluntarily and understand that functional lab panels may reveal patterns that I can discuss with my licensed medical provider.

Consent to Purchase Lab  ::  I authorize the ordering of this wellness lab panel through a physician authorization network (associated Lab Corp. laboratory). I understand that once the requisition is submitted, the purchase is non-refundable.

Confidentiality and Privacy  ::  My personal and health information will be kept confidential to the extent permitted by law.

Acknowledgment  ::  By purchasing any ThyroSisters service, I confirm that I have read, understand, and consent to the above terms.

 

TERMS OF PURCHASE

By completing this purchase, you acknowledge and agree that:

  1. The Thyroid Awakening Panel is a wellness-oriented, non-diagnostic functional lab panel.
  2. You are purchasing this lab for educational purposes and understand it is not medical advice.
  3. No doctor–patient relationship is created by this purchase or through subsequent consultations.
  4. All interpretations are educational explanations of lab patterns, not diagnoses.
  5. You remain solely responsible for seeking medical evaluation and treatment from a licensed provider.
  6. Once the lab requisition is submitted, the purchase is non-refundable.
  7. You must complete the Thyroid Awakening Health History Form and sign the Informed Consent before your results will be reviewed with you.
  8. Dr. Labbe may provide lifestyle, nutritional, and supplement education, but does not diagnose or treat medical conditions or adjust medications.


PRIVACY & CONFIDENTIALITY  ::  I understand that all personal information, health history, lab results, and communications that I share with Dr. Labbe or the ThyroSisters team will be kept private and confidential to the fullest extent permitted by law.

Although these wellness services do not constitute medical treatment and therefore are not subject to the federal HIPAA law in the same manner as a medical provider, Dr. Labbe voluntarily follows HIPAA-aligned privacy standards to safeguard my information.

My information will not be disclosed to any third party without my written permission, unless required by law. Secure systems may be used for communication, scheduling, chart storage, and payment processing.

I acknowledge that electronic communication (email, SMS, online forms, virtual sessions) may have inherent privacy risks, and I consent to receive communication through these methods for convenience.
