By completing this purchase, you acknowledge and agree that:

• The Thyroid Awakening Panel is a wellness-oriented, non-diagnostic functional lab panel.
• You are purchasing this lab for educational purposes and understand it is not medical advice.
• No doctor–patient relationship is created by this purchase or through subsequent consultations.
• All interpretations are educational explanations of lab patterns, not diagnoses.
• You remain solely responsible for seeking medical evaluation and treatment from a licensed provider.
• Once the lab requisition is submitted, the purchase is non-refundable.
• You must complete the Thyroid Awakening Health History Form and sign the Informed Consent before your results will be reviewed with you.
• Dr. Labbe may provide lifestyle, nutritional, and supplement education, but does not diagnose or treat medical conditions or adjust medications.

PRIVACY & CONFIDENTIALITY

I understand that all personal information, health history, lab results, and communications that I share with Dr. Labbe or the ThyroSisters team will be kept private and confidential to the fullest extent permitted by law.

Although these wellness services do not constitute medical treatment and therefore are not subject to the federal HIPAA law in the same manner as a medical provider, Dr. Labbe voluntarily follows HIPAA-aligned privacy standards to safeguard my information.

My information will not be disclosed to any third party without my written permission, unless required by law. Secure systems may be used for communication, scheduling, chart storage, and payment processing.

I acknowledge that electronic communication (email, SMS, online forms, virtual sessions) may have inherent privacy risks, and I consent to receive communication through these methods for convenience.
